Have you ever signed up for CPV? Is this typical?

Hi all, trying to get setup with CPV to try some of Anik Singal's latest techniques. In his webinar, directCPV is used. So, I tried signing up with them. Normally, I do not even give out my SS#, but with the IRS getting, uh, hmmm, more serious, you see it.

But, now, they have sent an email requiring extreme stuff for my credit card beyond what was entered onsite, and I do not feel comfortable about that!

In order to verify your deposit we require that you send in the attached credit card authorization form. We also need copies of the front and back of your credit card showing your signature, a copy of a valid ID such as a driver's license, and a proof of your billing address (if it is not the same as on your ID, a bill of some sort should be fine).
It smacks of a phishing scheme to me! Has anyone here also signed up with directCPV (and/or other CPV outfits)? Is this okay? Is it standard practice? Are they afraid of something because they are not completely legit?
