There will be many people bidding to webmd, your ads shows up when your keyword matches or if you bid on the url your ad shows when someone opens webmd

It would shows when he input the full domain like webmd.com, or webmd.com/info

But typically not when they inputs a sub-domain like health.webmd.com

But every PPV Networks handles these things differently, so make sure you contact them and ask. Also read over their FAQs.

Based off your example - It will show up when they visit the domain and not the keywords

But normally when a user search weight loss, he may use only keyword weightloss rather than webmd.com. Right? So how the inner pages get traffic. I am afraid some one type webmd.com/diet/default.htm‎ while surfing internet

People are not likely to type in deep link URLs like "webmd.com/diet/default.htm", but when someone navigates to that page it could trigger your ad.

When you target keywords (and not domains), it usually pops your ad for any URL that contains those keyword. This can bring more traffic, but it's usually less targeted.
