Background image in opt-in form - Wordpress Site

Hello guys, do you know some plugin that I can insert an opt-in form with a background image, like in the right side of this image: hxxp://i.imgur.com/txGLdTi.png

You can check how this plugin scrolls the screen in the right sidebar of this site: hxxp://comodefiniroabdomen.net

Do you know what plugin is it?

      1. Put your image as the background image in the widget.
      2. Put your code in for the autoresponder as a transparency that will overlay where the image is (that same widget area in the sidebar.
