How to add a Opt in Form on Facebook

I have gone through Youtube and the instructions cannot be followed as the Facebook has changed.
Could some one give me step by step instruction how to add Opt In form with free offer i would like to add to my Facebook Page.
Instruction should apply to the current page editing format.
Please i am sick of trying everything with Aweber forms and it is not working at all.

    Hi Laurie

    You can only add optin forms to a Facebook Custom Tab. I use templates to do this on mine and they're very simple to use.

    If you check out the Recommended Resources section on my website (link below) there is a link to the templates I use. Once you sign up for them you can set up custom tabs on your Facebook Page and add opt in forms. They come with some really good videos showing you step by step how to use them and how to add the optin form.
