Problem with Payment Processor Lead Collection - Brick Wall

I recently purchased a WP Theme called Product Pages that allows you to set up products and integrated with paypal. When a product is purchased the name and email is also collected. However I have been trying to get it to work for nearly a week and it will not collect the email and name no matter what.
I've exhausted their support, they won't help anymore, and so I am trying to work out from the debug long what is wrong. Apparently one of the guys in the FB group said look for something like:
"SUCCESS :autoresponder response...", "SUCCESS Product Email successfully sent to xxx@yyy.com" or "SUCCESS :Notify Email successfully sent to xxx@yyy.com"
I can't see this in the debug long so wondered in the off chance of posting in here someone might be able to see something I can't or be able to suggest what might be the cause. Or suggest a better theme, one that works!
So the debug log is below. I've replaced personal info with [name] etc. and IP addresses with xxx.
Hopefully I can get some suggestions as to what might be wrong here. Everything seems to be working perfectly except lead collection. There is a section in the page where you design the page and put your associated autresponder code, which has to have name and email fields.
