Anyone know this theme or plugin?

I'm trying to figure out a way to do what this lady is doing with her photos:

When you hover the image, a small description comes up along with a place to click to go to the link. I've spent about 20 hours trying different plugins and themes to mimic it, but I can't. Can anyone tell me how to do this or point me in the right direction? I would be extremely thankful.

Theme:
The theme is a custom theme created by magniumthemes, it's an adaptation of the piemont theme:

Piemont - Premium WordPress Personal Blogging Theme

You can generally check for themes with free online theme checkers like wpthemedetector.com but realistically all these guys do is scan the <url>/wp-content/themes folder.

The Wordpress theme is:

The plugin for the photo grid is:

I am not sure if the plugin for the Jackson photo grid is bundled with the theme. You would need to raise the question with the theme developer on ThemeForest, but I suspect that it isn't.
